Hot Potatoes
This is actually a collection of tools that enables a user to "create interactive multiple-choice, short-answer, jumbled-sentence, crossword, matching/ordering and gap-fill exercises for the World Wide Web" (Hot Potatoes Website). This software is completely free for non-profit educational institutions.
Quandary
From the makers of Hot Potatoes, Quandary allows a user to create "Action Mazes" (basically, interactive problem solving flow charts) that can be used for many purposes "including problem-solving, diagnosis, procedural training, and surveys/questionnaires" (Quandary Website). This software is not free, but a limited demonstration version is available.
